Thank you for taking part. This survey has two parts. Part 1 keeps our parish records current, so it asks for your name and contact information. Part 2 is the anonymous planning survey and asks for your honest opinions. Please answer what you can — there are no wrong answers, and you may skip any question.
This online version sends both parts together to the parish office. If you would like your Part 2 opinions to remain completely anonymous, simply leave the Part 1 name and contact fields blank, or use the printed copy and return the two parts separately.

The parish is exploring whether to build a columbarium — a sacred structure of niches for the dignified placement of cremated remains on parish grounds. No decision has been made, and diocesan guidelines require parishioner input before such a project can move forward.
